With the CEREC 3D method there is usually no need for impressions or a 2-3 week wait for your restoration to come back from the lab. You don’t have a second visit that might requiring numbing the area again because the procedure is completed in a Single Visit!!
Your damaged tooth is prepared for the CEREC 3D restoration which might be an inlay, on lay, crown or veneer. An optical impression is taken by simply photographing the tooth with a special computer camera. The restoration is then designed on the computer screen while you watch. Once the design is done, the computer instructs the milling portion of the CEREC 3D machine to form a solid block of color matched Ceramic/Porcelain to fit your tooth. The finished restoration is ready to be bonded into your tooth to complete the repair just 30-40 minutes later!
Missed Appointments
Our primary goal is to assist your in attaining and maintaining the best oral health. Therefore, your appointment time is reserved exclusively for you. We trust that no change in your appointment will be necessary and we will send you a postcard a week in advance and will call you 24 hours in advance to confirm your reserved time. Should something occur and you are unable to keep that scheduled time, please give the office 28-48 hours notice. If you fail to show for three appointments or give less than 24 hours notice for a cancellation, you may be dismissed from the practice.
PATIENT RIGHTS
You have the right to look at or get copies of your health information, with limited exceptions. You may request that we provide copies. You must make a request in writing to obtain access to your health information. You may request access by sending us a letter to the address at the end of this notice. If you request copies, we will charge you a reasonable cost-based fee that may include labor, copying costs, and postage.
Financing
While providing the very best of dentistry, we strive to keep our fees fair. Our fees are structured like many private dental practices. When treatment is rendered, we ask that you pay at the time services are provided.
Please understand that we file dental insurance as a courtesy to our patients. We do not have a contract with your insurance company, only you do. We are not responsible for how your insurance company handles its claims or for what benefits they pay on a claim. We can only assist you in estimating your portion of the cost of treatment.

Dental treatment is an excellent investment in an individuals medical and psychological well being. Financial considerations should not be an obstacle to obtaining this important health service. Being sensitive to the fact that people have different needs in fulfilling their financial obligations, we are providing the following options:

You can finance 100% of your dental treatment with no upfront costs, no annual fees, and no pre-payment penalties. With the popular No Interest Payment Plans there are no interest charges if you pay your balance in full within the specified time period. We also offer a revolving credit line for additional treatment By financing, you are making a sensible investment in your dental work.
Change in Medical History
At your initial visit and each following visit to our office; we will review your medical and dental health history. To protect your health, please advise us any changes in your medical history, changes in or addition of prescriptions/other medications, any surgeries or hospitalizations since your last dental visit.
